Preparing for Adulthood for young people with SEN/disabilities
Transition social workers with contribute to the EHC planning process due to be implemented and young people likely to require adult services are identified and monitored as they approach the relevant age group through a variety of processes, including referrals from CYPLD teams to the transition team.
Transition social workers are allocated to work with families and soon as possible after referral is received to aid future planning.
We have a variety of operational and strategic process in place to improve the general quality of support to young people in transition, such as joint planning (monthly) and SEN (fortnightly) panels, transition team meetings (fortnightly), transition partnership board (quarterly), transition operational group (quarterly).
We will liaise with colleagues in education/schools, children's social care, health and other partners in ensuring effective and comprehensive support in meeting social care needs.
Assessment and support planning documents lead transition social workers to use an outcome-focused approach when considering how best to meet the needs of young people in transition, particularly with reference to promotion of independence.
The transition team has link-workers in the adult mental health and the LD clinical health teams to improve multi-disciplinary approaches and develop practice in preparing for adulthood between these teams.
